RELEASE AND WAIVER

I acknowledge that participating in the DFI Equipment Rodeo is potentially hazardous due to the nature of the activities which involve the use of construction equipment as well as other construction tools and processes. Knowing the foregoing and in consideration for being allowed to participate in this event, I hereby release the Deep Foundations Institute (DFI), and all sponsors and including their agents, employees, assigns or anyone acting for or on their behalf, from any and all claims or liability for death, personal injury or property damage of any kind or nature whatsoever arising out of, or in the course of, my participation in this event though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. This Release and Waiver extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown.

The undersigned grants full permission to DFI to use any photographs, videotapes, motion pictures recording, or any other record of my participation in this event for promotional purposes.

Participation in the event by minors will only be accepted if this document is signed by the minor's parent or guardian. If a parent or guardian is signing on behalf of a minor, then the parent or guardian agrees to defend and indemnify all parties listed in the first paragraph of this release and waiver against any claim brought against them by that minor at any time, arising out of the minor's participation in this event.
